You can handle images on Hubzilla the Mastodon way and attach them to posts as files. But then you won't have any way of adding or editing alt-text, and the original file name will be the alt-text.
The only way to add alt-text is the blog way: You upload the image to the file space included in your Hubzilla channel. Then you embed the image in your post. And then you have to manually work the alt-text into the image-embedding code.

However, this is not officially documented anywhere. Hubzilla users only learn about this through word-of-mouth.
Also, this means that you will have to manually edit naked code with no WYSIWYG live preview. Even Hubzilla users aren't always happy about having to get their hands dirty on cryptic, not-toot-self-explanatory code and "program" alt-text into their posts.
But Hubzilla has also got one big advantage over Mastodon:

Mastodon normally allows for only 500 characters for the post and 1,500 for alt-text. Thus, Mastodon users tend to put everything into the alt-text, even if it doesn't belong there, e.g. explanations.
Hubzilla doesn't limit its post length. If you've got something to explain, you can do so in the post, no matter how much you have to explain. So there can be a short visual description in the alt-text and an extensive one in the post.
